SJS and TEN Treatment and Medical Care

Early Stevens-Johnson syndrome management

Early diagnosis, identification of the drug potentially causing SJS or toxic epidermal necrolysis (TEN)—for example, Dilatin or Lamictal—and its discontinuation are vital for recovery. Fatality rates increase proportionately with the late withdrawal of drug use. Also, use of SJS-related drugs subsequent to recovery may cause SJS reoccurrence, usually with a more severe allergic reaction the next time the drug is used.

Medical specialists

SJS or TEN diagnoses must be confirmed by a dermatologist. Other medical professionals, such as burn specialists or plastic surgeons, may be needed for skin care. If SJS affects the mucous membranes of other organs—lungs, GI tract, eyes, etc.—other specialists such as pulmonologists, gastroenterologists, or ophthalmologists may be needed.

Symptom management

General treatment of milder symptoms such as itching, swelling, and infection may include the following:

  • Bed rest
  • Antibiotics
  • Painkillers
  • Mouthwashes
  • Sedatives
  • Antihistamines
  • Topical creams
  • Anesthetics

There is no treatment or cure that stops the progression or SJS or TEN. But, treatment can reduce symptoms and the reaction duration, lessen danger of complications, and ease discomfort.

Infection prevention

If the condition is severe, hospitalization in a burn unit or intensive care area is usually required. In many respects, caring for SJS or TEN patients involves the same procedures used for burn patients.

Patients are at risk for fluid loss and bacterial infection. Possible treatments that help keep the patient hydrated, fed, and uninfected include the following:

  • Proper temperature control to prevent heat loss
  • Adhesive material avoidance
  • An isolated sterile environment
  • Pain medication
  • IV and gastro-intestinal feeding

Permanent scarring, disability, and vision problems are some of the aftermaths that SJS or TEN patients face.
